چکیده
bifascicular block (RBBB + LAFB). Interrupted episodes of complete paroxysmal AV block were detected during electrocardiographic monitoring on the ward. Echocardiography showed mild left ventricular systolic dysfunction with inferior akinesis. During implantation of a two-chamber permanent pacemaker via the left axillary vein, we observed LSVC persistence, the characteristics of which were similar to those described in the previous case. Two active fixation leads with distal electrodes were implanted in RA and RVOT, using manually preformed curved stylets identical to those described in case 1. No dislocations or changes in the stimulation parameters were observed during follow-up. Persistent LSVC can make implantation of a permanent pacemaker difficult. The fact that the coronary sinus ostium is not aligned with the tricuspid annulus means that the main technical problem consists of inserting the lead in the RV. The RVA has been the site of choice for cardiac stimulation, but reaching it in these cases usually means that the probe electrode forms a big loop in the RA. Stimulation in the RVOT has been proposed as an alternative to RVA stimulation. In terms of lead stability, it has shown similar long-term results to those achieved with stimulation at the apex,

A Series of Unfortunate Events: Prinzmetal Angina Culminating in Transmural Infarction in the Setting of Acute Gastrointestinal Hemorrhage
Prinzmetal angina or vasospastic angina is a clinical phenomenon that is often transient and self-resolving. Clinically it is associated with ST elevations on the electrocardiogram, and initially it may be difficult to differentiate from an acute myocardial infarction. The vasospasm induced in this setting occurs in normal or mildly to moderately diseased vessels and can be triggered by a numbe...

OBJECTIVE To examine the association between a lifetime history of migraines and other headaches with and without aura and Rose angina and coronary heart disease (CHD). METHODS Participants were 12,409 African American and white men and women from the Atherosclerosis Risk in Communities Study, categorized by their lifetime history of headaches lasting > or =4 hours (migraine with aura, migrai...

Background: The sporadic type of Hemiplegic Migraine (HM) is sometimes observed among migrainous patients (MP) and mimics ischemic strokes. Methods: In an evaluation of two-hundred consecutive adult MP in the Mackenzie headache clinic, Canada during 2004 , 9% of the patients met the criteria established by the International Headache Society for sporadic HM. Female to male sex ratio, family...

متن کاملAchromatopsia in the aura of migraine.
A 49 year old woman reported an attack of transient neurological dysfunction associated with unilateral headache. A prominent feature of the aura was a period of complete achromatopsia, so that the visual scene was experienced in monochrome. The episode developed to include features of prosopagnosia and spatial agnosia before resolving completely. Other episodes of transient neurological dysfun...
